Workers Comp, Disability and Unemployment Claims
If you become sick, disabled or unemployed due to COVID-19, you may be eligible for Workers Compensation, Disability Benefits or Unemployment Insurance.
If you contracted COVID-19 at your work, such as a health care worker, notify your employer immediately and file workers' compensation form C-3 with the NYS Workers Compensation Board.
If you contracted COVID-19 away from work, file disability benefits form DB-450. If your job was deemed non-essential and you were laid off, you can apply for Unemployment Insurance from the NYS Dept. of Labor.
If you are under an order of mandatory or precautionary quarantine or isolation for COVID-19 issued by a governmental agency, like the Dept of Health, file disability benefits/PFL form SCOVID. If your minor child is under an order of mandatory or precautionary quarantine or isolation for COVID-19, you would file disability benefits/PFL form CCOVID. And if you are caring for a family member who was been diagnosed with COVID-19, you would file PFL form PFL-CARE.
If you must stay at home to care for your children because school was closed, you can learn about your eligibility for paid sick leave from your employer under the Federal Emergency Paid Sick Leave Act.
